GAPP Services

Georgia Pediatric Program (GAPP)

This program was created to aid parents with the assistance
of their child’s everyday living needs; Feeding assistance, bathing,
incontinence care, transfer assistance, grooming, getting dressed, G-tube care,
medication administration, etc. This care will be provided by one of our
licensed professionals.

The In-Home Skilled Nursing Program is designed to serve
eligible members based on a medical necessity determination for children under
the age of 20 years 11 months. We are here to make a much better environment
for you and your child by ensuring that they receive the support they need.

No out of pocket In-Home Care for children and young adults
under 21.

-Is your child under 21?

-Does your child need help with everyday living activities
ie.; feeding assistance, incontinence care, transfer assistance, bathing, etc.?

-Does your child have Medicaid?

If so, then your
child may qualify for the no out-of-pocket home care services.

We provide no out of pocket In-Home private care services to
children under the age of 21, who may have significant developmental and/or
physical disabilities.

Special Needs Children

Our services are a great solution for families that want professional service for their child, who may be in need of assistance with
ADL’s and/or their everyday living arrangements.

This program gives the caretaker assistance with the care of the child on a daily basis; including help out of bed, grooming, dressing assistance, and a nutritious breakfast.

Assistance can be provided every step of the way so that the
day gets started right and ends with a good night’s rest.

Our professional staff can also assist with evening hygiene
routines and daily ROM exercises both in and out of therapy equipment to ensure
a restful night.

These services can be beneficial to a healthier lifestyle for
both the child and the parent.

Our Private Home Care Service:

* SOFA Homecare is an approved Medicaid program.
* We are a part of GAPP (Georgia’s Pediatric Program), a Medicaid funded program.
* We specialize in both skilled and unskilled private In-Home care.

This program is not a waiver service. It allows
your child to receive services immediately following approval.

Our intake and evaluation can take up to 30 days for final approval.

This program allows the parents to be involved with the development of their child’s service plan of treatment, along with any service goals.

This program was created to service both children in need of assistance with skilled and unskilled needs.
Our professional staff are all Licensed RN's, LPN's, and certified/ or trained, CNA's, HHA's, and PHA's.

To apply for the GAPP program, Perspective clients have to Apply through the SOFA agency. Complete the Request Form and upload it on the website.
